A sample measure of 1.5 inches, comprising approximately 200 hair strands (roughly the width of a #2 pencil) nearest to the scalp, yields 100mg of hair, which is the optimal sample for analysis and validation. For EtG, additional checks, and/or examinations exceeding 10 panels, a 150mg sample is advisable. It is suggested to weigh the sample on a jeweler’s scale. If scalp hair is not present, a comparable quantity of body hair can be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair categories (such as facial, axillary, etc.).

Process Overview

The primary stages in the laboratory procedure for processing a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample within a laboratory’s framework. This consists of checking that the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and concluding any further data entry absent in an electronic chain of custody system.

Screening pertains to the initial swift assessment for drug misuse. Though Screening offers a budget-friendly method to exclude drug usage on the majority of samples, a positive screen must be verified to be legally permissible. Samples suspected to be positive based on Screening necessitate subsequent confirmation.

If a sample is suspected to be positive during the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. During this stage, drugs are extracted from hair at a significantly lower concentration compared to other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most complex methodology to execute.

Confirmation of any positive screening finding is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All assumed positive samples are cleansed prior to confirmation as necessary. The complete laboratory sequence from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
