For EtG, extended panels, or tests exceeding 10 panels, a sample of 150mg is advised. It's important to measure the specimen using a jeweler’s scale. Should scalp hair be unattainable, a comparable quantity of body hair is acceptable. Head hair specifically pertains to scalp hair, while body hair encompasses other types (such as facial, axillary, etc.).

Process Overview

The procedure for analyzing a drug test sample in a laboratory consists of four vital stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning handles the preliminary processing of a sample into the lab's system. This step includes ensuring the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and filling in any missing information that an electronic chain of custody system doesn't provide.

Screening serves as a preliminary assessment for detecting substance abuse. Though Screening is a cost-efficient approach for dismissing drug use in most samples, any positive results require confirmation to be valid in legal settings. Samples testing positive during Screening must undergo secondary validation.

If a sample tests positive during Screening, additional hair from the original specimen is set aside for Extraction. In this stage, substances are extracted from hair at significantly lower concentrations compared to other methods (like urine or oral fluid), which makes hair drug screening the most intricate to perform.

Confirmation of any positive Screening findings is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo a washing process before confirmation if needed. The end-to-end lab procedure, from Accessioning to Confirmation, is supervised under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
