What it is, How it works

Hair analysis is recognized as an effective method for identifying drug and alcohol consumption. Hair captures a detailed history of alcohol and other substances by retaining biomarkers in the strands as they grow. When cut close to the scalp, hair can offer a detection period of roughly three months for alcohol and drugs. It is easy to gather, difficult to tamper with, and straightforward to transport.

A 1.5-inch piece, consisting of about 200 hairs (roughly the width of a #2 pencil) nearest to the scalp, will yield 100mg, which is the perfect sample for both screening and verifying. For EtG, extra tests, and/or tests beyond 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale to weigh the sample. Should scalp hair be unavailable, an equal volume of body hair can be collected. When discussing head hair, we specifically mean scalp hair. Body hair signifies all types aside from scalp hair (facial, axillary, etc.).

Process Overview

The laboratory processes a drug test in four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a sample upon entry into a lab system. This step ensures the sample is correctly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any ancillary data entry not included in an electronic custody system.

Screening is an initial, swift evaluation for drug abuse. Though Screening is an economical method to invalidate drug use in most samples, a positive result must be verified to be used in legal proceedings. Samples that test positive in Screening must undergo a subsequent confirmation.

When a Screening test shows a presumptive positive, more hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are withdrawn from hair at a much lower concentration than other methods (e.g., urine or oral fluid), rendering hair drug testing one of the most challenging methods.

Confirmation of a positive Screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positive results are washed before confirmation, if necessary. The complete lab process—from Accessioning to Confirmation—is evaluated under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
