For EtG, supplementary tests, and/or assessments exceeding 10 panels, 150mg of the sample is advised. It is suggested to weigh the sample using a jeweler’s scale. In the absence of scalp hair, an equivalent volume of body hair can be obtained. When mentioning head hair, it solely refers to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The four predominant procedures in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a specimen within a lab’s framework. This comprises ensuring the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any remaining data entry not managed by an electronic chain of custody system.

Screening involves a swift preliminary assessment for drugs of abuse. While Screening offers a cost-effective means to exclude drug usage in most samples, a positive result necessitates confirmation to be considered in legal proceedings. Any samples initially testing positive in Screening require a subsequent confirmation.

If a specimen shows a preliminary positive result in the Screening phase, additional hair is extracted from the original sample and prepared for Extraction. During this phase, drugs are removed from hair at significantly lower concentrations compared to alternate methodologies (e.g., urine or oral fluid), making hair drug testing one of the most intricate procedures to undertake.

Confirmation of any positive initial results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All potentially positive samples undergo washing before confirmation, as necessary. The entire laboratory workflow from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair certification and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
