What it is, How it works

Hair stores a comprehensive record of drug and alcohol in its growing strands by embedding biomarkers. By gathering hair close to the scalp, it provides a detection timeline of up to three months for various substances. Hair collection is straightforward, challenging to tamper with, and simple to send for examination.

A 1.5-inch sample containing around 200 strands (similar to the dimensions of a #2 pencil) taken closest to the scalp offers 100mg of hair, which is ideal for initial testing and verification. For EtG, additional screenings, and/or panels exceeding 10, a sample weighing 150mg is preferred. It is advisable to weigh the sample using a jeweler’s scale. In cases where scalp hair is not available, a similar quantity of body hair can be used. References to head hair imply scalp hair just. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for drug test result processing includes the stages of Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the sample's initial integration into the laboratory's system. This stage confirms proper sealing and shipping of the sample, allocates a random LAN (Laboratory Accessioning Number), and wraps up any missing data entry from an electronic chain of custody system.

Screening offers an initial rapid evaluation for substance abuse. While this step is a budget-friendly way to eliminate drug presence in most samples, positive results in Screening require verification to be legally accepted. Samples yielding a presumptive positive in Screening need secondary confirmation.

If a sample exhibits a presumptive positive in Screening, additional hair is extracted from the initial collection and processed for Extraction. In this stage, hair is analyzed at a lower drug concentration level compared to other methods (e.g., urine or saliva), which attributes to its complexity in execution.

Any positive findings from Screening are validated through GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples with presumptive positives are cleansed before confirmation if needed. The full laboratory cycle from Accessioning through Confirmation adheres to both CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditation.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
