Hair testing is regarded as a potent method for detecting drug and alcohol consumption. Hair provides an extensive history of alcohol and drug use by trapping biomarkers within the fibers of the growing hair strand. When collected near the scalp, hair can offer a detection window of approximately three months for alcohol and drugs. Hair is straightforward to collect, relatively hard to tamper with, and simple to transport.

A 1.5-inch section of around 200 hair strands (roughly the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, the perfect amount for screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of the sample is advised. It's recommended to measure the sample using a jeweler’s scale. In the absence of scalp hair, a similar amount of body hair can be collected. By head hair, we mean only scalp hair. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

There are four primary steps in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a specimen within a laboratory's system. This process includes verifying that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not provided by an electronic chain of custody system.

Screening carries out an initial rapid assessment for drugs of abuse. Although Screening is a cost-effective means to rule out drug use in most samples, a positive screen must be confirmed to be admissible in court. Any samples deemed presumptively positive in Screening do require further confirmation.

If a sample is deemed presumptively positive during Screening, additional hair is extracted from the initial sample and prepped for Extraction. In this stage, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which explains why hair drug screening is the most challenging method to perform.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptively positive samples are cleansed prior to confirmation as necessary. The complete laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
