For EtG, add-ons, and/or over 10-panel tests, 150mg of the sample is advised. It is recommended to weigh the sample using a jeweler's scale. If scalp hair is not available, an equivalent quantity of body hair can be collected. The term head hair refers solely to scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a sample into the lab's system. This step includes verifying the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening constitutes an initial rapid assessment for drugs of abuse. While it serves as a cost-efficient method for excluding drug use in most samples, positive screenings must be corroborated to stand up in court. Any samples that appear positive in Screening require a secondary confirmation.

If a sample tests positive in the Screening phase, further hair is extracted from the initial sample and prepared for Extraction. In this step, substances are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug testing is the most challenging method to undertake.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptive positive are washed before confirmation as necessary. Every aspect of the lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
