For EtG, additional analyses, and/or tests exceeding 10 panels, a specimen weight of 150mg is advised. Use a jeweler’s scale to weigh the specimen. In the absence of scalp hair, a similar quantity of body hair can be taken. Head hair refers specifically to scalp hair. Body hair encompasses all other types such as facial, axillary, etc.

Process Overview

The laboratory processing of a drug test result entails four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning focuses on the initial intake of a sample into the lab’s workflow. This step encompasses confirming that the sample was securely sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and entering any additional information not available via an electronic chain of custody system.

Screening involves a preliminary drug check. Though Screening is a financially efficient method to exclude drug use for most specimens, positive results must be verified for court admissibility. Samples testing presumptively positive in Screening undergo secondary confirmation.

Should a specimen show presumptive positivity in the Screening phase, extra hair is extracted from the original sample for preparation during Extraction. This phase involves extracting drugs from hair at significantly lower concentrations compared to other testing methods (such as urine or oral fluid), making hair drug testing the most challenging method to execute.

Positive screening outcomes undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed prior to confirmation when required. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
