A 1.5-inch hair sample consisting of approximately 200 strands (about the width of a #2 pencil) from the scalp provides 100mg of hair, the optimal amount for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale for weighing the sample. If scalp hair is not available, an equivalent amount of body hair can be collected. When mentioning head hair, we specifically refer to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary phases in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of processing a sample into the lab’s system. This step involves confirming that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening is a preliminary quick examination for drug abuse. Though Screening is an economical method to dismiss drug usage for most samples, any positive screen must undergo confirmation to be acceptable in court. Samples that test presumptively positive in Screening require further confirmation.

If a sample shows signs of positivity in the Screening stage, additional hair is extracted from the original specimen and prepared for Extraction. During this process, drugs are extracted from hair at a much lower concentration than in other methodologies (e.g., urine or oral fluid), highlighting why hair drug testing is the most challenging method to execute.

Confirmation of any positive Screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive undergo washing prior to confirmation as necessary. The complete lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
