What it is, How it works

Hair testing has gained recognition as an effective method for detecting drug and alcohol consumption. Hair serves as a long-term record of alcohol and drug use by capturing biomarkers within the strands as they grow. Collected near the scalp, hair can reveal alcohol and drug use over a span of about three months. Hair is easy to collect, relatively resistant to tampering, and simple to transport.

Obtaining a 1.5-inch sample of approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields a 100mg sample, which is optimal for both initial screening and confirmation. For EtG, additional tests, and instances requiring analysis beyond ten panels, a 150mg sample is advisable. We recommend using a jeweler's scale to weigh the sample. If scalp hair is not accessible, an equivalent amount of body hair may be used. 'Head hair' specifically refers to scalp hair, whereas 'body hair' encompasses all other types (such as facial or axillary hair).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the laboratory system. This step ensures the sample was properly sealed and shipped, assigns it a random LAN (Laboratory Accessioning Number), and completes any additional data missing from an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. Although Screening is a cost-efficient approach to exclude drug use in most samples, any positive result requires confirmation to be legally accepted. Samples that screen as presumptively positive must undergo a secondary confirmation.

In instances of a presumptive positive in Screening, more hair is extracted from the original sample and prepared for Extraction. During this stage, drugs are extracted from hair at a much lower concentration than in alternative methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology.

Positive results in Screening are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed prior to confirmation as necessary. The process from Accessioning to Confirmation adheres to the CAP (College of American Pathologists) Hair designation and aligns with the standards of ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
