Process Overview
There are four principal steps in the laboratory analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.
Accessioning covers the initial integration of a sample into a laboratory's system. This step includes ensuring the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any supplemental data entry absent from an electronic chain of custody system.
Screening entails an initial rapid assessment for illicit drugs. While Screening is a cost-effective means to exclude drug use in the majority of samples, a positive screening must be confirmed for it to hold up in court. Any samples that yield a presumptive positive in Screening need secondary confirmation.
When a sample yields a presumptive positive in the Screening phase, additional hair is extracted from the initial sample for further processing in the Extraction phase. In this phase, drugs are extracted from hair at substantially lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug testing a particularly challenging technique to execute.
Any positive results from Screening are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptive positive are cleansed before confirmation as required. The entire laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.
The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
