What it is, How it works

A 1.5-inch portion consisting of about 200 hair strands (approximate thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal quantity for both screening and confirmation. For EtG, additional panels, and/or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair is not present, a comparable quantity of body hair may be gathered. By head hair, we imply scalp hair solely. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The four primary phases in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a sample within a laboratory system. This process includes confirming that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finalizing any additional data input not facilitated by an electronic chain of custody system.

Screening conducts an initial assessment for substances of abuse. While Screening is a cost-efficient means to exclude drug presence in most samples, a positive screen must be affirmed to be valid in court. Samples that are tentatively positive during Screening require further confirmation.

If a sample is tentatively positive at the Screening stage, more hair is extracted from the original specimen and prepared for Extraction. In this phase, substances are removed from hair at substantially lower concentrations than other methods (e.g., urine or oral fluid), thereby making hair drug screening one of the most challenging methodologies.

Confirmation of any positive screening outcome is executed through GC/MS, GC/MS/MS or LC/MS/MS. All tentatively positive samples are cleansed before confirmation if necessary. The complete lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
