Process Overview

The laboratory handling of a drug test outcome encompasses four chief steps: Accessioning, Screening, Extraction, and Confirmation.

This comprises confirming the sample was sealed and dispatched accurately, assigning a random LAN (Laboratory Accessioning Number), and performing any extra data entry absent from an automated chain of custody process.

Screening constitutes an initial rapid assessment for drugs of abuse. Though Screening is a cost-efficient approach to negate drug use in the majority of cases, a positive screen necessitates confirmation for court admittance. Any samples that show a presumptive positive in Screening mandate a secondary confirmation.

Should a sample show presumptive positivity during the Screening phase, additional hair is extracted from the original sample and prepped for Extraction. During this phase, drugs are drawn from hair at a reduced concentration compared to alternative methodologies (such as urine or oral fluid), making hair drug screening one of the most complex methodologies to execute.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptively positive samples are washed as necessary. The entire laboratory progression from Accessioning to Confirmation is subjected to review under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
