Process Overview
The four primary stages in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.
During Accessioning, the sample undergoes initial processing in the laboratory system. This step verifies that the sample was appropriately sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes additional data entry not handled by an electronic chain of custody system.
Screening conducts an initial rapid assessment for drugs of abuse. While it is a cost-effective method to exclude drug use in the majority of specimens, a positive result from Screening must be validated for court admissibility. Samples that are presumptively positive during Screening necessitate a secondary confirmation.
Should Screening suggest a presumptive positive, more hair is extracted from the initial collection for Extraction. This phase extracts drugs from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid). Thus, hair drug testing is the most challenging methodology to execute.
Confirmation for any positive screen result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples undergo washing if required. The entire lab process, from Accessioning through Confirmation, is scrutinized under both CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
