Hair analysis has gained recognition as an effective means for identifying drug and alcohol consumption. Hair serves as a prolonged history of alcohol and drugs by embedding biomarkers within the strands of growing hair. When taken near the scalp, hair offers up to an approximately 3-month timeframe for detecting alcohol and drugs. Collecting hair is straightforward, challenging to tamper with, and easy to transport.
A 1.5-inch segment of around 200 hair strands (similar to a #2 pencil) nearest to the scalp yields 100mg of hair, which is ideal for examination and verification. For EtG, add-ons, or tests exceeding 10 panels, 150mg of the sample is suggested. We advise using a jeweler’s scale to measure the sample. If scalp hair is unavailable, a similar quantity of body hair can be obtained. Reference to head hair means scalp hair specifically, while body hair covers all other types (facial, axillary, etc.).
Process Overview
The main steps in laboratory handling of a drug test result include Accessioning, Screening, Extraction, and Confirmation.
Accessioning encompasses the initial integration of a sample into a lab’s system. This involves ensuring the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any further data entry absent in an electronic chain of custody system.
Screening entails a preliminary evaluation for abusive substances. While Screening is an economical approach to rule out drug use on most samples, a positive result must be confirmed for court acceptability. Any samples that are preliminarily positive in Screening necessitate a secondary confirmation.
If a sample is presumptively positive during Screening, more hair is taken from the original specimen and readied for Extraction. Here, drugs are extracted at a lower concentration from hair compared to other techniques (e.g., urine or oral fluid), making hair drug analysis a highly challenging methodology.
Confirmation of any positive screening outcome is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as required. The entire lab procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
