When collected near the scalp, hair offers an insight into substance use over the past three months. Hair sampling is easy to conduct, hard to tamper with, and straightforward to transport.

A sample measuring 1.5 inches, consisting of roughly 200 hair strands (akin to the diameter of a #2 pencil) nearest to the scalp, yields 100mg of hair, which is ideal for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg specimen is preferred. Utilizing a jeweler’s scale for weighing the specimen is advisable. If scalp hair is not accessible, a similar quantity of body hair can be utilized. References to head hair pertain solely to scalp hair, whereas body hair includes all other types (such as those found on the face, underarms, etc.).

Process Overview

The laboratory procedure for processing a drug test outcome is comprised of four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a lab’s system. This step entails ensuring the sample was properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and adding any data not captured by an electronic chain of custody system.

Screening offers a preliminary evaluation for substances of abuse. While it provides a cost-efficient method for ruling out drug use in most samples, positive results need subsequent confirmation to be court-admissible. Samples showing presumptive positives in Screening necessitate an additional confirmation step.

In cases where the sample appears presumptively positive during Screening, more hair is extracted from the initial sample for the Extraction stage. During this stage, drugs are isolated from hair at significantly lower levels than in other methods (e.g., urine or oral fluid), explaining why hair drug screening is particularly challenging to execute.

Confirmation of any positive during Screening is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicating presumptive positives undergo a cleansing process before confirmation when necessary. The comprehensive lab procedure from Accessioning through Confirmation is conducted under the CAP (College of American Pathologists) Hair accreditation and ISO / IEC 17025 standards.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Novel Analytical Techniques: The Verona meeting witnessed the unveiling of cutting-edge analytical techniques, offering enhanced sensitivity, selectivity, and accuracy for drug testing. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
