What it is, How it works

When collected near the scalp, hair can reveal up to approximately a 3-month period of drug and alcohol activity. Collecting hair is straightforward, challenging to tamper with, and simple to transport.

A 1.5-inch piece encompassing approximately 200 hair strands (about the width of a #2 pencil) closest to the scalp yields 100mg of hair, the perfect amount for testing. For EtG, additional testing, and/or panels above 10, 150mg of hair is suggested. We advise using a jeweler’s scale for accurate weighing. If scalp hair isn't available, a similar amount from body hair can be used. When mentioning head hair, we specifically mean scalp hair. Body hair pertains to all other types of hair (facial, axillary, etc.).

Process Overview

The procedure for processing a drug test result in the laboratory includes four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the starting point of processing a sample into the lab's framework. This process ensures the sample is correctly sealed and shipped, assigns an arbitrary LAN (Laboratory Accessioning Number), and completes any extra data input not covered by an electronic custody system.

Screening involves a preliminary swift assessment for drug presence. While Screening is cost-effective for dismissing drug usage across most samples, a positive screen needs verification to be valid in legal proceedings. Any samples preliminarily positive during Screening require further confirmation.

If a sample tests preliminary positive in the Screening phase, additional hair is extracted from the initial sample and set up for Extraction. In this step, drugs are drawn from hair at significantly lower concentrations than other methods (like urine or oral fluid), making hair drug screening the most complex method to conduct.

Confirmation of any positive results from screening is done using GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples preliminarily positive are washed before confirmation if necessary. The complete laboratory procedure, from Accessioning to Confirmation, is evaluated in line with the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
