What it is, How it works

Hair analysis has emerged as an effective method for identifying drug and alcohol consumption. Hair captures long-term evidence of substances within the strands as they grow. When sourced near the scalp, hair tests offer up to roughly a 90-day window of detection for substances. Collecting hair is straightforward, hard to tamper with, and simple to transport.

An ideal screening and confirmation sample involves a 1.5-inch segment of roughly 200 strands (comparable to a #2 pencil’s thickness) nearest to the scalp, yielding 100mg of hair. For EtG, addon tests, or panels exceeding 10, a 150mg sample is advisable. It is suggested to measure the sample with a jeweler’s scale. In the absence of scalp hair, equivalent body hair can be collected. "Head hair" pertains solely to scalp hair, while "body hair" denotes all other types (e.g., facial, axillary).

Process Overview

Drug testing through laboratory methods involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial sample processing into the lab system. This step involves ensuring proper sealing and shipment, generating a random LAN (Laboratory Accessioning Number), and completing any missing data entry tasks aside from what's supplied by an electronic custody system.

Screening is an initial rapid assessment for substances misuse. Although Screening is cost-effective for many samples, a positive result requires confirmation for legal admissibility. Any screenings that suggest positive results must undergo further confirmation.

Should a Screening test suggest a positive, additional hair is taken from the initial collection and prepared for Extraction. This step detects drugs in hair at much lower levels than through other means (such as urine or saliva), making it a particularly challenging method.

For confirming a positive screening, GC/MS, GC/MS/MS, or LC/MS/MS methods are used. Pre-confirmation involves washing presumptive positive samples as necessary. The full journey from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair category and ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Long detection period: Hair analyses can reveal drug use spanning up to 90 days, in contrast to the shorter period seen in urine analyses.
  • Hard to manipulate: Cheating a hair analysis is quite challenging, ensuring more reliable results.
  • Offers usage patterns: Delivers an understanding of drug consumption over some time frame, not just recent usage.

Limitations:

  • Lacks recent use detection: Drug presence in hair typically takes about 5-7 days to become noticeable.
  • Expense: Hair screening tends to be more costly than other drug testing approaches.
  • Result variability: Factors like hair color and growth rate can influence metabolite levels in hair.

Note: Although often termed as "hair follicle tests", these tests assess the hair strand itself and not the follicle beneath the scalp.
