A 1.5-inch segment comprising roughly 200 hair strands (similar to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for screening and confirmation. For EtG, additional tests, and/or tests beyond 10 panels, 150mg of the specimen is advisable. It is suggested to weigh the specimen on a jeweler's scale. If scalp hair is unavailable, a comparable amount of body hair can be collected. When mentioning head hair, we refer exclusively to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

Laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial phase of processing a sample into a laboratory’s system, including verifying proper sealing and shipping of the sample, allocating a random LAN (Laboratory Accessioning Number), and completing any remaining data entry absent in an electronic chain of custody system.

Screening includes an initial quick check for drug abuse signs. Though Screening offers a cost-efficient method to exclude drug usage in the majority of samples, a positive screen necessitates confirmation to be admissible in judicial proceedings. Samples with presumptive positives in Screening require a follow-up confirmation.

When a sample yields a presumptive positive in the Screening phase, additional hair is extracted and prepared for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than found in other methodologies (e.g., urine or oral fluid), emphasizing why hair drug testing is among the most complex techniques to execute.

Confirmation of any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation is examined under CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
