What it is, How it works

The analysis of hair has evolved as a significant method for identifying drug and alcohol consumption. Hair offers a prolonged history of substance use by capturing biomarkers in the strands of hair as it grows. If sampled near the scalp, hair can provide an approximately 3-month detection timeframe for drugs and alcohol. Collecting hair is straightforward, relatively tamper-resistant, and convenient to transport.

A sample of 1.5 inches with roughly 200 hair strands (approximately the thickness of a #2 pencil) close to the scalp provides 100mg of hair, which is the optimal quantity for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a specimen of 150mg is suggested. We advise verifying the weight of the specimen using a jeweler’s scale. In the absence of scalp hair, an equivalent amount of body hair can be used. When discussing head hair, we mean scalp hair strictly. Body hair refers to all other types (such as facial or axillary hair).

Process Overview

The laboratory processing of drug test results consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a sample into a laboratory’s system. This process includes checking that the sample was correctly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry missing from an electronic chain of custody system.

Screening is an initial examination to detect drug abuse. Although Screening is an economical approach to exclude drug use in most samples, a positive screen must be validated to be valid in legal proceedings. Samples that test presumptive positive in Screening require subsequent confirmation.

When a sample tests as presumptive positive during the Screening phase, additional hair is taken from the initial sample for Extraction. During Extraction, drugs are extracted from hair at significantly lower concentrations than other testing methods (like urine or oral fluid), which makes hair drug screening the most challenging method to execute.

The Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptive positive are prepped by washing before confirmation if required. The entire laboratory procedure from Accessioning to Confirmation undergoes review in accordance with the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
