Hair secures a long-term history of alcohol and other substances by embedding biomarkers in the fibers of the developing hair strand. Collected near the scalp, hair can present up to about a 3-month detection period for alcohol and other substances. Hair is easy to collect, relatively hard to alter, and simple to send.

A 1.5-inch portion of roughly 200 hair strands (comparable in size to a #2 pencil) closest to the scalp delivers 100mg of hair, the optimal sample size for screening and confirmation. For EtG, additional tests, and/or those above 10 panels, 150mg of the specimen is advised. We suggest weighing the specimen with a jeweler’s scale. If scalp hair is not available, a similar amount of body hair can be collected. References to head hair pertain exclusively to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The drug test result processing in a laboratory involves four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial entry into a laboratory’s system. This process verifies the sample's sealing and shipping integrity, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody system.

Screening provides an initial quick examination for drugs of abuse. Screening is a cost-effective method to eliminate drug use in most samples; however, a positive screen must be confirmed for courtroom admissibility. Samples with presumptive positive results in Screening require a further confirmation.

With a presumptive positive result in Screening, more hair is extracted from the original specimen and prepared for Extraction. During this stage, drugs are extracted from hair at lower concentrations than other methods (like urine or oral fluid), making hair drug screening a challenging methodology.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as needed. The entire laboratory process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
