What it is, How it works

When taken near the scalp, hair offers a detection window of up to roughly 3 months for alcohol and drugs. It is easy to gather, hard to tamper with, and convenient to transport.

A 1.5-inch section of approximately 200 hair strands (similar to the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, an optimal sample for both screening and confirmation. For EtG, additional tests, or panels exceeding 10, a 150mg specimen is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair is not an option, an equivalent amount of body hair can be collected. Head hair strictly refers to scalp hair, whereas body hair includes all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test consists of four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the laboratory's database. This step includes ensuring the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening is a preliminary check for drug substances. It is a cost-efficient method to negate drug use in most samples, though a positive screen requires confirmation for legal admissibility. All samples that screen positive require additional confirmation.

Should a sample be positive at the Screening phase, more hair is extracted from the initial specimen for preparation in the Extraction phase. During this step, drugs are removed from hair at a significantly lower concentration than in other methods (like urine or oral fluid), making hair drug testing the most challenging to execute.

To verify positive screening results, tests are performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results undergo washing before confirmation when necessary. The complete laboratory process from Accessioning to Confirmation is conducted under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
