A 1.5-inch segment comprising roughly 200 strands of hair (akin in size to a #2 pencil) closest to the scalp yields 100mg of hair, the optimal sample for analysis and verification. For EtG, additional tests, and/or panels surpassing 10, 150mg of the specimen is advised. It's best to weigh the sample using a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair may be collected. Referring to head hair means scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four primary steps in the laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample into a lab's system. This includes checking that the sample was securely sealed and correctly shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of misuse. While Screening is a cost-efficient approach to exclude drug use on the majority of samples, a positive result in Screening must be confirmed for acceptance in court. All samples that present a presumptive positive in Screening necessitate further confirmation.

If a sample presents a presumptive positive at Screening, more hair is extracted from the original specimen and readied for Extraction. In this phase, substances are isolated from hair at notably lower concentrations compared to other methods (such as urine or oral fluid), hence hair drug evaluation is the most intricate method to execute.

Confirmation of any positive Screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing as needed before Confirmation. The entire laboratory method from Accessioning to Confirmation is assessed under both CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
