What it is, How it works

A segment of 1.5 inches with about 200 hair strands (resembling the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample for screening and confirmation. For EtG add-ons or tests exceeding 10 panels, it is suggested to use 150mg of the specimen. The specimen should ideally be weighed using a jeweler’s scale. If scalp hair is unavailable, a comparable quantity of body hair is accepted. When head hair is mentioned, it exclusively refers to scalp hair. Body hair signifies all other types (facial, axillary, etc.).

Process Overview

The critical steps in laboratory processing of drug test results are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where the sample is integrated into the laboratory’s system. This phase ensures the sample’s proper sealing and shipping, assigns a unique LAN (Laboratory Accessioning Number), and completes any supplementary data entry absent from an electronic chain of custody system.

Screening is a preliminary assessment for drugs of abuse. While Screening is a cost-efficient avenue to eliminate drug usage in most samples, any positive screen mandates confirmation for legal admissibility. Samples that yield a presumptive positive during Screening necessitate a secondary verification.

If a sample tests presumptively positive at the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. In this phase, drugs are isolated from hair at much lower concentrations than in other methods (like urine or oral fluid), which contributes to the complexity of performing hair drug screening.

Positive screening results require Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo pre-confirmation washing as needed. The lab process from Accessioning to Confirmation is scrutinized under both CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
