A sample of 1.5 inches containing about 200 hair strands (roughly the width of a #2 pencil) nearest to the scalp yields 100mg, which is the ideal amount for testing and verification. For EtG add-ons or tests exceeding 10 panels, a specimen of 150mg is suggested. The specimen's weight should be verified with a jeweler’s scale. If scalp hair is lacking, a similar quantity of body hair can be taken. Head hair refers solely to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The lab process for a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial handling of a sample within a lab system. This step involves confirming the sample’s proper sealing and shipment, assigning it a random LAN (Laboratory Accessioning Number), and completing any additional required information not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for substances of abuse. While Screening effectively eliminates drug use in most samples, a confirmed positive result is necessary for legal proceedings. Any samples that test preliminarily positive during Screening must undergo secondary verification.

If a sample turns presumptively positive in Screening, additional hair is taken from the specimen for Extraction. This stage extracts drugs from hair at much lower concentrations than other methods (e.g., urine or oral fluid), highlighting why hair drug testing is the most challenging to execute.

Any positive screening outcomes are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positivity are washed prior to confirmation as required. The complete lab procedure, from Accessioning to Confirmation, undergoes a review under both the CAP (College of American Pathologists) Hair certification and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
