What it is, How it works

When taken close to the scalp, hair may offer approximately a 90-day detection window for alcohol and substances. It is easy to gather, somewhat challenging to tamper with, and simple to transport.

A 1.5-inch segment of about 200 hair strands (roughly the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for screening and confirmation. For EtG, add-ons, and/or analyses exceeding 10 panels, a specimen of 150mg is advised. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair can be used. In context, head hair refers to scalp hair only, while body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The primary steps involved in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample into the laboratory’s system. This involves checking that the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent in an electronic chain of custody system.

Screening is an initial rapid check for drugs of abuse. Although Screening is a cost-efficient approach to eliminate drug use in most samples, a positive screen must be verified for court admissibility. Any samples that are presumptively positive in Screening require a secondary confirmation.

If a sample is preliminarily positive at the Screening phase, additional hair is extracted from the original specimen and readied for Extraction. At this phase, drugs are isolated from hair at a considerably lower concentration than in other methods (e.g., urine or oral fluid), making hair drug screening the most intricate method to conduct.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS or LC/MS/MS. Every presumptive positive sample undergoes washing prior to confirmation as needed. The entire laboratory procedure from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.
