Yet, for recent drug consumption within 5 days, urine tests remain more precise.

What it is, How it works

Hair analysis has emerged as an effective method for detecting drug and alcohol use. Hair offers an extended record of alcohol and drugs by embedding biomarkers in the fibers of the growing hair strand. When collected near the scalp, hair can show up to roughly a 3-month detection timeframe for alcohol and other substances. Hair is easy to gather, hard to tamper with, and simple to transport.

A sample of 1.5 inches comprising about 200 hairs (approximately the girth of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal sample size for testing and corroboration. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the specimen is suggested. It's best to weigh the specimen on a jeweler’s scale. If scalp hair is not available, an equal volume of body hair can be collected. By head hair, we specifically mean scalp hair. Body hair includes all other hair types (facial, underarm, etc.).

Process Overview

The lab processing of a drug test involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a specimen into a laboratory’s system, which involves verifying that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not supplied by an electronic chain of custody system.

Screening is a rapid initial review for drugs of abuse. Although Screening is a cost-effective means to rule out drug use on most samples, any positive screen must be confirmed to be admissible in legal settings. Samples that are presumptively positive in Screening require additional confirmation.

If a sample is presumptively positive at the Screening phase, more hair is extracted from the original specimen for the Extraction process. In this phase, drugs are extracted from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug screening is considered the most challenging method to execute.

Confirmation of any positive screening result is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as needed. The entire lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
