What it is, How it works

Hair assessments are increasingly recognized as an effective means for identifying substance and alcohol consumption. Hair captures a prolonged record of alcohol and drug activity by embedding biomarkers within the threads of the hair as it grows. When obtained near the scalp, hair offers roughly a three-month examination timeframe for alcohol and drugs. Collecting hair is straightforward, moderately resistant to tampering, and convenient for transport.

A sample measuring 1.5 inches, consisting of about 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal quantity for analysis and validation. For EtG, additional tests, or more than 10-panel evaluations, 150mg of the sample is advisable. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is not available, an equivalent quantity of body hair can be gathered. By head hair, we mean scalp hair exclusively. Body hair pertains to all other hair varieties (facial, axillary, etc.).

Process Overview

The four primary stages in the laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample within a laboratory's system. This step verifies proper sealing and shipping of the sample, assigns a unique LAN (Laboratory Accessioning Number), and completes additional data entry if not included in an electronic chain of custody system.

Screening involves a preliminary assessment for substances of abuse. While Screening offers a cost-efficient means to dismiss drug use in most samples, a positive result must be confirmed for legal admissibility. Any samples indicating a presumptive positive in Screening require further confirmation.

If a sample shows a presumptively positive result in the Screening phase, additional hair is taken from the original sample for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug testing the most complex technique to conduct.

Confirmation of any positive screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleaned before confirmation as necessary. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
