A 1.5-inch segment of around 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample size for both screening and confirmation. For tests including EtG, supplementary panels, or beyond 10 panels, 150mg of the hair sample is advised. It is suggested to use a jeweler’s scale to measure the specimen. If scalp hair cannot be obtained, an equivalent volume of body hair is an alternative. When we mention head hair, it specifically pertains to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial intake of a sample into the laboratory’s system. This process ensures the sample was securely packaged and dispatched, generates a random LAN (Laboratory Accessioning Number), and includes any remaining data entry not managed through an electronic chain of custody process.

Screening represents a preliminary examination for potential drug misuse. Although Screening provides an economical approach to exclude drug use in most samples, a positive result necessitates confirmation for legal considerations. Samples that show presumptive positivity in Screening do require a subsequent confirmation step.

When a sample is deemed presumptively positive during Screening, additional hair is extracted from the original sample and processed for Extraction. This stage involves isolating drugs from hair at much lower concentrations than other methods (such as urine or oral fluid), making hair drug tests among the most challenging methodologies to execute.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positives are cleansed prior to confirmation when needed. The comprehensive laboratory workflow from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
