What it is, How it works

Hair testing is increasingly recognized as a potent method for detecting drug and alcohol use. Hair captures a prolonged record of alcohol and drug consumption by embedding biomarkers within the fibers of a growing hair strand. A sample collected near the scalp can offer a detection period of up to approximately three months for alcohol and drugs. Hair collection is straightforward, relatively resistant to tampering, and convenient to transport.

A 1.5-inch sample, consisting of roughly 200 hair strands (comparable to the size of a #2 pencil) nearest the scalp, yields 100mg of hair, which is considered an optimal sample for screening and verification. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the sample is advised. It's suggested to measure the sample using a jeweler’s scale. In the absence of scalp hair, a comparable quantity of body hair can be collected. When mentioning head hair, it specifically refers to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The processing of drug test results in the laboratory comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial processing of a sample into the lab's system. This step includes ensuring the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening entails a preliminary rapid examination for substances of abuse. While Screening is a cost-efficient method to exclude drug consumption in most samples, a positive screen necessitates confirmation for court admissibility. All samples tentatively positive in Screening require an additional confirmation.

If a sample shows tentative positivity in the Screening phase, more hair is extracted from the initial specimen and prepared for Extraction. In this phase, substances are removed from hair at a significantly lower concentration compared to other methodologies (e.g., urine or oral fluid), explaining why hair drug screening is the most challenging methodology to implement.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All tentative positive samples are cleansed before confirmation as required. The comprehensive lab process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
