For EtG, additional tests, and those beyond 10 panels, 150mg of hair is advisable. We advocate weighing the specimen on a jeweler's scale. If scalp hair is lacking, an equal quantity of body hair can be used. In this context, head hair specifically refers to scalp hair, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory analysis of drug test results includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a sample into the lab system. This step entails verifying the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing necessary data entries not covered by an electronic chain of custody system.

Screening constitutes a primary quick assessment for drugs of abuse. While economical in eliminating drug use in most samples, positive screens need confirmation for courtroom admissibility. Samples showing presumptive positives in Screening require subsequent confirmation.

When a sample tests presumptively positive during Screening, additional hair is extracted from the original sample for the Extraction phase. In this stage, drugs are extracted from hair at a significantly reduced concentration compared to other methods (like urine or oral fluid), marking hair drug screening as a particularly challenging method.

Confirmation of any positive result occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed prior to confirmation as required. The overall lab process from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
