If scalp hair is not available, the same amount of body hair can be gathered. When mentioning head hair, it pertains solely to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial integration of a sample into the laboratory's system. This step verifies that the sample was sealed and delivered correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry absent from an electronic chain of custody system.

Screening serves as a preliminary rapid assessment for drug abuse. Although Screening is a cost-efficient method to eliminate drug use in most samples, a positive screen must be confirmed to be valid in legal settings. Samples that are tentatively positive in Screening necessitate a further confirmation.

If the sample is tentatively positive during Screening, additional hair is extracted from the initial sample and prepped for Extraction. During this phase, drugs are extracted from hair at lower concentrations than other methods (e.g., urine or oral fluid), which renders hair drug screening a challenging method to execute.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are washed before confirmation as required. The entire laboratory operation from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
