A sample consisting of 1.5 inches of roughly 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp produces 100mg of hair, which is the optimal quantity for screening and confirmation. For EtG, extra tests, and/or over 10 panels, 150mg of the specimen is suggested. It's advisable to weigh the specimen using a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair can be utilized. Head hair specifically means scalp hair only. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory testing procedure for drug evaluation consists of four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample enters a laboratory’s workflow. This encompasses confirming the sample was properly sealed and transported, allocating a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry missing from an electronic chain of custody system.

Screening involves an initial rapid analysis for drugs of abuse. Although Screening is a cost-efficient strategy to eliminate drug usage for most samples, a positive screen must be substantiated to be legally valid. Samples that initially test positive during Screening must undergo secondary confirmation.

If a sample tests presumptively positive at the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. In this phase, drugs are isolated from hair at much lower concentrations than in alternative testing methods (e.g., urine or oral fluids), hence hair drug screening is considered a more challenging methodology.

Positive screening confirmations are performed using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples that are presumptively positive undergo washing before confirmation as necessary. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation standards.
