What it is, How it works

Hair testing has emerged as an influential method for identifying drug and alcohol usage. Hair offers a comprehensive history of alcohol and drug intake by encapsulating biomarkers within the strands of growing hair. When sourced near the scalp, hair provides a detection timeframe of up to about 3 months for drugs and alcohol. It is easy to collect, relatively hard to tamper with, and simple to transport.

An ideal sample size is obtained from a 1.5-inch section with around 200 hair strands (similar in width to a #2 pencil) nearest the scalp, yielding approximately 100mg of hair for screening and confirmation. For EtG, add-ons, or tests exceeding 10 panels, a 150mg specimen is suggested. We advise using a jeweler's scale to weigh the sample. Should scalp hair be unavailable, an equivalent volume of body hair can be used. "Head hair" refers specifically to scalp hair, while "body hair" encompasses all other types (such as facial and axillary hair).

Process Overview

The primary elements in the laboratory procedure of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a lab's system, ensuring the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any requisite data entry unless provided by an electronic custody system.

Screening serves as an initial swift evaluation for drugs of abuse. Though Screening is cost-effective for excluding drug use in most samples, a positive screening outcome needs confirmation to be court-admissible. Samples appearing presumptively positive in Screening require an additional confirmation step.

Upon a sample being presumptively positive at the Screening stage, additional hair is extracted from the original sample and processed for Extraction. Here, drugs are extracted from hair at a substantially lower concentration than other methods (e.g., urine or oral fluid), which makes hair drug testing the most intricate methodologically.

The Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive findings undergo washing before confirmation as deemed necessary. The entire sequence from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair criteria and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
