If head hair is not accessible, a comparable mass of body hair can be used. References to head hair specifically denote scalp hair. Body hair encompasses all other hair forms (such as facial, underarm, etc.).

Process Overview

The four key phases in processing a drug test result in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes processing a sample into the lab’s system initially. This step involves validating that the sample was securely sealed and properly transported, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any remaining data entry not covered through an electronic chain of custody system.

Screening entails an initial rapid evaluation for illicit substances. Any samples that indicate presumptive positivity during Screening require further confirmation.

Here, drugs are isolated from hair at a concentration significantly lower than in alternative techniques (such as urine or oral fluid), presenting a challenge in hair drug screening practices.

Positive Screening outcomes must undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed as necessary before confirmation. The full lab process, from Accessioning to Confirmation, is overseen following both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
