What it is, How it works

For EtG, additional assessments, and extensive panels (over 10), 150mg is advocated. It's advisable to measure the sample using a jeweler’s scale. If scalp hair isn't available, a comparable mass of body hair can be used. When mentioning head hair, it pertains to scalp hair exclusively. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory procedures for determining a drug test result encompass four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with incorporating a sample into the lab's framework. This step involves ensuring the proper sealing and shipment of the sample, generating a random LAN (Laboratory Accessioning Number), and completing any data entry not already present in an electronic chain of custody system.

Screening offers an initial rapid assessment for substance misuse. Screening serves as a cost-efficient method to exclude drug use for most samples, though positive results require confirmation for judicial admissibility. Samples deemed presumptively positive in Screening necessitate further validation.

Should a sample register as presumptively positive during Screening, additional hair is extracted from the original sample and readied for Extraction. During this step, drugs are isolated from hair at concentrations lower than those in other methods (e.g., urine or oral fluid), underpinning the complexity of hair drug testing.

Confirmation for any positive screening findings is executed through GC/MS, GC/MS/MS, or LC/MS/MS. Preconfirmation, all presumptively positive samples undergo washing as part of necessary procedures. The entire laboratory sequence, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair directive and the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
