What it is, How it works

A 1.5-inch segment consisting of nearly 200 strands of hair (approximately the thickness of a #2 pencil) closest to the scalp yields 100mg, which is optimal for screening and confirmation. For EtG, additional panels, and/or tests beyond 10 panels, a specimen of 150mg is advised. It is suggested to measure the specimen using a jeweler’s scale. If scalp hair is unavailable, an equivalent quantity of body hair can be used. When referring to head hair, we mean only scalp hair, while body hair encompasses other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a lab's system. This procedure verifies that the sample was securely sealed and properly shipped, assigns it a random LAN (Laboratory Accessioning Number), and completes any additional data entry not included by an electronic chain of custody system.

Screening involves an initial cursory check for drugs of abuse. While Screening is an economical approach for eliminating drug use for the majority of samples, a positive screen requires confirmation to be valid in legal settings. Samples that show a presumptive positive in Screening need further confirmation.

If a sample shows a presumptive positive in the Screening phase, additional hair is obtained from the initial specimen and set for Extraction. At this point, drugs are extracted from hair at much lower concentrations than other methodologies (e.g., urine or oral fluid), which makes hair drug screening the most challenging approach to execute.

Confirmation of any positive screening outcome is done using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are washed as needed. The entire laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
