What it is, How it works

Hair analysis is recognized as an effective method for identifying drug and alcohol consumption. The hair captures biomarkers within its fibers as it grows, providing an extensive record of substance use. When taken near the roots, hair can offer a detection timeframe of roughly three months for alcohol and drugs. It's easy to collect, challenging to tamper with, and straightforward to transport.

Obtaining a sample of 1.5 inches consisting of about 200 hair strands (roughly the size of a #2 pencil) closest to the scalp yields 100mg, which is the optimal amount for testing. For EtG, additional requests, and tests beyond 10 panels, a sample of 150mg is advisable. We suggest using a jeweler’s scale to weigh the sample. If head hair is not available, an equivalent amount of body hair can be used. When mentioning head hair, we mean solely the hair on the scalp. Body hair encompasses all other types (such as facial or armpit hair).

Process Overview

The laboratory analysis of a drug test follows four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial incorporation of a specimen into the lab's system. This process involves checking that the specimen was appropriately sealed and sent, assigning it a random LAN (Laboratory Accessioning Number), and completing any missing information not included in an electronic chain of custody system.

Screening consists of an initial swift examination for substance abuse. Although Screening is an economical method to exclude drug use for most samples, a positive screening outcome must be verified for legal acceptance. Any samples that are preliminarily positive in Screening necessitate a secondary confirmation.

In cases where a sample shows a presumptive positive during Screening, additional hair is extracted from the original specimen and prepared for Extraction. At this phase, drugs are removed from hair at significantly lower concentrations compared to other methods (like urine or oral fluids), making hair drug screening one of the most complex methodologies to execute.

Verification of any positive screening result is done through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positive results are cleansed before confirmation as required. The full laboratory procedure from Accessioning to Confirmation undergoes evaluation following both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
