When taken close to the head, it can reveal alcohol and drug usage history for up to about three months. Hair collection is straightforward, challenging to tamper with, and easy to transport.

A sample spanning 1.5 inches consisting of roughly 200 hair strands (around the same size as a #2 pencil) closest to the scalp yields 100mg, considered the optimal amount for analysis and verification. For EtG, additional tests, and tests exceeding 10 panels, a collection of 150mg is advised. It's suggested to measure the sample with a jeweler's scale. If scalp hair can't be obtained, a comparable amount of body hair can be used. In terms of head hair, it specifically means scalp hair. In contrast, body hair encompasses all other hair categories (beard, armpit, etc.).

Process Overview

The procedure to process a drug test in a lab involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of incorporating a sample into the lab's protocol. It involves checking that the specimen was properly sealed and transported, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data input that might not come from an electronic custody tracking system.

Screening serves as a preliminary assessment for drug traces. Although Screening is an economical tactic to eliminate drug usage in most samples, confirmation is essential for any positive screen to hold up in legal contexts. Samples flagged as potentially positive in Screening require additional confirmation.

A sample seen as potentially positive in the Screening process will have additional hair retrieved from the original collection and set up for Extraction. This phase includes removing drugs from the hair at significantly lower levels than other treatment methods (e.g., urine or oral fluids), thus why hair drug testing is notoriously challenging to conduct.

Confirming any positive findings from Screening happens through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are cleaned before confirmation as needed. The overall lab process, from Accessioning to Confirmation, is under the scrutiny of both the CAP (College of American Pathologists) Hair standards and meets the ISO / IEC 17025 accreditation criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
