For EtG additions, and/or assessments beyond 10 panels, a recommendation of 150mg of the sample is advised. Using a jeweler’s scale for precise measurement is recommended. If scalp hair is unavailable, one may collect an equivalent quantity of body hair. When speaking of head hair, it is specific to scalp hair only. Body hair accounts for all other types (such as facial, axillary, etc.).

Process Overview

The four fundamental phases in the laboratory procedure of a drug test result comprise Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the step of initially logging a sample into a laboratory’s database. This step ensures the sample has been properly sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and completes supplementary data entry not covered by an electronic custody chain system.

Screening involves a preliminary rapid assessment for substances of abuse. While Screening is an economical approach to exclude drug usage in most samples, a positive outcome necessitates confirmation for judicial admissibility. Any samples that screen positive on a preliminary basis require a further confirmation process.

Should a sample register positive during Screening, additional hair is extracted from the initial specimen for the Extraction phase. Here, drugs are meticulously extracted at lower concentrations than other methods (e.g., urine or oral fluid), underscoring why hair drug screening presents a greater challenge in execution.

Confirmation for any positive screening outcome is conducted via GC/MS, GC/MS/MS, or LC/MS/MS methods. Prior to confirmation, all initially positive samples undergo washing as necessary. The entire laboratory sequence from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair criteria and conforming with ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
