Hair testing is now regarded as a powerful method for detecting the use of drugs and alcohol. Hair offers a long-term record of alcohol and other substances by catching biomarkers in the strands of growing hair. When taking hair close to the scalp, it can reveal up to roughly a 3-month window for detecting alcohol and drugs. Hair is easy to obtain, somewhat tough to falsify, and convenient to mail.
A 1.5-inch piece of about 200 hair strands (similar in size to a #2 pencil) nearest the scalp provides 100mg of hair, the optimal sample for both screening and confirmation. For EtG, additional tests, and/or assessments over 10 panels, 150mg of specimen is advised. We suggest weighing the sample on a jeweler’s scale. If head hair cannot be used, a similar quantity of body hair may be taken. When mentioning head hair, it refers strictly to scalp hair. Body hair includes all other types (facial, axillary, etc.).
Process Overview
The four primary steps in lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.
Accessioning involves the sample's initial processing into a laboratory’s system. This stage ensures the sample was sealed and sent correctly, assigns a random LAN (Laboratory Accessioning Number), and handles any further data input not offered by an electronic chain of custody system.
Screening includes a preliminary swift check for drugs of abuse. While Screening is an economical approach to negate drug use in most samples, a positive result must be verified to be admissible in legal contexts. Any samples that are presumptively positive in Screening need a further confirmation.
If a sample shows presumptively positive at the Screening stage, additional hair is taken from the initial specimen and prepped for Extraction. In this phase, drugs are extracted from hair in a significantly lower concentration compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging method to execute.
Confirmation of any preliminary positive screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if required. The complete laboratory process from Accessioning to Confirmation is overseen according to both the CAP (College of American Pathologists) Hair guideline and the accreditation to ISO / IEC 17025 standards.
