What it is, How it works

When harvested near the scalp, hair offers a detection scope extending up to roughly 3 months for alcohol and other substances. Hair gathering is straightforward, moderately cumbersome to tamper with, and conveniently deliverable.

A 1.5-inch section comprising roughly 200 hair strands (comparable to the thickness of a #2 pencil) proximate to the scalp yields 100mg, constituting the optimal sample for evaluation and verification. For EtG, supplementary analyses, and/or assessments exceeding 10 panels, a specimen weighing 150mg is advised. Specimen measurement should be conducted using a jeweler’s scale. If scalp hair is unattainable, an equivalent portion of body hair may be obtained. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The drug testing process encompasses four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the introductory procedure of registering a sample into a lab’s framework. This step confirms the sample’s proper sealing and dispatch, assigns an arbitrary LAN (Laboratory Accessioning Number), and finalizes any residual data input absent from an electronic chain of custody setup.

Screening executes a preliminary rapid assessment to identify substance misuse. While economical in ruling out drug use for most samples, a positive screening necessitates confirmation to be legally admissible. Samples that indicate a presumptive positive in Screening must undergo subsequent confirmation.

If a sample registers a presumptive positive during Screening, additional hair is extracted from the initial specimen and prepped for Extraction. In this procedure, drugs are extracted from hair at a significantly lower concentration than with other methods (e.g., urine or oral fluid), rendering hair drug screening the most challenging method to execute.

Positive screening confirmations are managed through GC/MS, GC/MS/MS, or LC/MS/MS means. All presumptive positive specimens undergo washing pre-confirmation as required. Each stage of laboratory processing—spanning from Accessioning to Confirmation—is scrutinized under both the CAP (College of American Pathologists) Hair accreditation and the ISO / IEC 17025 standard.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
