A 1.5-inch sample comprising about 200 hair strands (roughly equal to the circumference of a #2 pencil) from near the scalp yields 100mg, which is the ideal quantity for screening and validation. For EtG, add-ons, or tests beyond 10 panels, 150mg of the specimen is advised. We suggest measuring the specimen with a jeweler's scale. If scalp hair is not available, an equivalent amount of body hair can be used. "Head hair" refers exclusively to scalp hair. "Body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of entering a sample into the lab’s system. This includes ensuring the sample is properly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing any missing data entry not captured by an electronic chain of custody.

Screening is a preliminary scan for drugs of abuse. While it is an economical approach to dismiss drug use in most samples, a positive screening must be confirmed for court purposes. Samples that test presumptively positive in the Screening stage need further confirmation.

If a sample is presumptively positive during Screening, additional hair is extracted from the sample and readied for Extraction. At this stage, drugs are isolated from the hair in much lower concentrations compared to other techniques (e.g., urine or oral fluid), which makes hair drug testing a challenging method to execute.

Confirmation for any positive screening is done using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples testing presumptive positive are washed as necessary. The complete laboratory process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.
