For EtG, additional tests, and/or analyses exceeding 10 panels, 150mg of the sample is advisable. Measuring the specimen on a jeweler's scale is recommended. Should scalp hair be absent, an equivalent portion of body hair might be collected. The term head hair refers specifically to scalp hair, while body hair pertains to all other types of hair (facial, axillary, etc.).

Process Overview

The four primary steps in the lab processing cycle of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample into a laboratory’s system. This process verifies that the sample was sealed and transported correctly, assigns a unique LAN (Laboratory Accessioning Number), and completes any remaining data entry absent from an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. While Screening is an economical approach to exclude drug use in most cases, a positive screen requires confirmation for court admissibility. Samples showing presumptive positive results during Screening need additional confirmation.

In cases where a sample tests presumptively positive during the Screening phase, additional hair is extracted from the original specimen and readied for Extraction. Here, drugs are retrieved from hair at lower concentrations compared to other techniques (e.g., urine or oral fluid), emphasizing the intricacy of hair drug testing.

Confirmation of positive screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples undergo washing as needed. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
