If scalp hair is not available, a comparable amount of body hair can be collected. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, underarm, etc.).

Process Overview

The laboratory processing of drug test results encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a lab’s system. This process involves ensuring the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finalizing any additional data input absent from an electronic chain of custody system.

Screening entails a preliminary rapid assessment for illicit drugs. Though Screening offers a budget-friendly method to negate drug usage in most samples, a positive result needs confirmation for legal acceptability. Samples showing tentative positives in Screening necessitate further validation.

When a sample is tentatively positive during the Screening phase, more hair is extracted from the original specimen and readied for Extraction. In this phase, drugs are extracted from hair at much lower concentrations compared to other methodologies (e.g., urine or oral fluid), making hair drug screening the most intricate methodology to perform.

Confirmation for any positive screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Every tentatively positive sample is cleansed before confirmation as required. The full laboratory procedure from Accessioning to Confirmation is assessed both under the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
