An ideal sample for screening and confirmation is a 1.5-inch segment of around 200 hair strands (about the thickness of a #2 pencil) closest to the scalp, providing 100mg of hair. For EtG add-ons and/or tests exceeding 10 panels, 150mg is advisable. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair isn't available, a similar quantity of body hair might be used. When mentioning head hair, we refer solely to scalp hair. Body hair denotes any other hair types (facial, axillary, etc.).

Process Overview

The primary four steps in a laboratory's drug test result processing involve Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample enters the laboratory's system. This stage ensures the sample was properly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data input absent in an electronic chain of custody system.

Screening represents an initial rapid assessment for drugs of abuse. Though Screening is a cost-effective method to dismiss drug usage in the majority of samples, a positive screen necessitates confirmation to be legally valid. Any samples with a presumptive positive result in Screening require secondary confirmation.

If a sample is presumptive positive during the Screening phase, additional hair is extracted from the initial specimen for the Extraction process. Here, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to conduct.

Confirmation of any positive screening outcome is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation if required. The complete laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
