What it is, How it works

Utilizing hair testing is recognized as a formidable means to identify drug and alcohol consumption. Hair offers a long-lasting record of alcohol and drug usage by capturing biomarkers within the fibers of the growing hair. When gathered from near the scalp, hair can provide a detection window of up to about 3 months for alcohol and other substances. Hair is straightforward to collect, somewhat challenging to tamper with, and simple to ship.

A sample measuring 1.5 inches, consisting of roughly 200 hair strands (about the size of a #2 pencil) nearest the scalp, yields 100mg of hair, the optimal sample size for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg specimen is advised. It's recommended to weigh the specimen using a jeweler’s scale. If scalp hair is not available, a similar amount of body hair may be used. By head hair, we mean scalp hair exclusively. Body hair pertains to all other types, such as facial or axillary hair.

Process Overview

The primary steps in processing a drug test outcome in the lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample within the lab's system. This step involves ensuring the sample was properly sealed and shipped, assigning it a random Laboratory Accessioning Number (LAN), and completing any further data entry omitted by the electronic chain of custody system.

Screening entails a preliminary swift check for substance misuse. While it is a cost-effective approach to eliminate drug usage for the majority of samples, a positive screening must be confirmed for court acceptance. All samples presumed positive in Screening undergo an additional confirmation step.

When a sample is tentatively positive in the Screening phase, additional hair is removed from the primary specimen and prepped for Extraction. Here, drugs are extracted from hair at a notably lower concentration than other methods (such as urine or oral fluid), highlighting the difficulty of hair drug testing.

Confirmation of any positive screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumed positive samples are washed if necessary. The laboratory procedure, from Accessioning to Confirmation, is validated under both the CAP's Hair designation and the accreditation standards of ISO / IEC 17025.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
