A sample of 1.5 inches, consisting of about 200 hair strands (roughly the diameter of a #2 pencil) nearest to the scalp, yields 100mg of hair, the ideal amount for both screening and confirmation. For EtG, additional tests, and tests exceeding 10 panels, 150mg of the sample is advised. We suggest using a jeweler's scale to weigh the specimen. If scalp hair is absent, a corresponding amount of body hair may be used. Our reference to head hair pertains solely to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The key stages in the lab process for reporting a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a lab’s system. It includes verifying the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry not covered by an electronic chain of custody.

Screening is a preliminary, cost-efficient method to rule out drug presence in most samples. However, a positive screen must be confirmed to be valid in court. Samples that are presumptively positive in Screening need further confirmation.

If Screening indicates a presumptive positive, more hair is drawn from the original specimen for Extraction. In this phase, drugs are removed from hair at lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most complex method to perform.

Confirmation of any positive screen result uses GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation if needed. The full lab process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
