A section of 1.5 inches containing roughly 200 hair strands (approximately the thickness of a #2 pencil) closest to the scalp provides 100mg of hair, which is the ideal amount for assessment and validation. For EtG, supplementary tests, and those exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair is missing, a similar quantity of body hair may be collected. When referring to head hair, it specifically means scalp hair. Body hair includes all other hair categories (facial, axillary, etc.).

Process Overview

The primary four steps in the laboratory handling of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample within a laboratory’s framework. This step includes confirming that the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data entry not supplied by an electronic chain of custody system.

Screening performs an initial assessment for drug utilization. Although Screening serves as an economical method to exclude drug use in most samples, a positive screen necessitates confirmation to be valid in legal scenarios. Any samples provisionally positive in Screening require further confirmation.

If a sample is provisionally positive during the Screening phase, additional hair is extracted from the initial specimen and set for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations compared to other methodologies (e.g., urine or oral fluid), which accounts for hair drug screening being the most challenging technique to conduct.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS or LC/MS/MS. All samples provisionally positive are cleansed before confirmation if required. The entire laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
