What it is, How it works

Hair analysis has gained recognition as an effective method for identifying drug and alcohol consumption. Hair acts as a repository for biomarkers, preserving a long-term record of alcohol and drug use in the strands as they grow. Collected near the scalp, hair can offer nearly a 3-month detection timeframe for substances. It is straightforward to obtain, relatively resistant to tampering, and simple to transport.

A sample measuring 1.5 inches from approximately 200 hair strands (roughly the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is ideal for screening and confirmation. For EtG, enhancements, and/or tests exceeding 10 panels, a specimen of 150mg is recommended. It is advisable to measure the specimen with a jeweler’s scale. If scalp hair cannot be obtained, an equivalent amount of body hair is acceptable. References to head hair pertain solely to scalp hair, whereas body hair refers to all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase, involving the processing of a sample into the laboratory’s database. This step verifies proper sealing and shipping of the sample, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional input not available from an electronic chain of custody.

Screening is a preliminary evaluation for substances of abuse. Although Screening is a cost-efficient approach to dismiss drug usage in most cases, a positive screen must be confirmed for legal acceptability in court. Samples testing presumptively positive in Screening require further confirmation.

Presumptively positive samples from the Screening phase require more hair from the original specimen for the Extraction phase. Here, drugs are extracted from hair at a much lower concentration compared to other techniques (e.g., urine or oral fluid), making hair drug testing the most challenging method to conduct.

Confirmation for any positive screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing prior to confirmation as necessary. The complete laboratory process, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
